I’m in Ohio and this is something I have dealt with often.
I would ask the hospital’s case manager and patient relations team for the proposed discharge destination and care plan, a list of SNF referrals and responses, and the process for challenging the discharge.

I see it happen a lot where they try to discharge somebody out even though they never gave the family a list of places to check out. I have been able to stall discharge until we looked at places as a client’s right issue to participate in their care planning and I have had luck.
