Trisha, I love your practice of watching for the initial comments, it always makes me smile when I see your response. I would want each client contracted separately. To expand on Brad’s response, the other concern is HIPAA. There may be information Mom might not want shared. Family dynamics are always interesting. ( insert eye roll )

I agree with Anne, it’s best to have all parties on the call. Patients typically don’t know this is an available option and spend valuable time going back and forth, and are left feeling frustrated and not getting the timely care they need. If the provider is part of the patients insurance network, I would call the insurance company and let them know what is happening. I have gotten the patient, the provider and payer on the phone at the same time so everyone knows what the rules are.

Half of the advance directive is appointing a HC-POA or Surrogate. I have not heard this. I agree with having discussions, but if you are clear about your wishes, having them in writing ensures your healthcare POA will follow your wishes.
