If the provider is part of the patients insurance network, I would call the insurance company and let them know what is happening. I have gotten the patient, the provider and payer on the phone at the same time so everyone knows what the rules are.

I have not heard this. I agree with having discussions, but if you are clear about your wishes, having them in writing ensures your healthcare POA will follow your wishes.
