I attend medical appointments with my geriatric clients and make sure prior to seeing a healthcare provider that they know what they must discuss and what questions should be asked. I also do in-hospital monitoring. I make sure they understand and are comfortable with the treatment plan. I communicate with their ALFs to ensure the doctor’s orders are being followed, and I check to make sure SNFs are taking appropriate steps to get medical issues treated in a timely manner. If I spot a potential problem, I contact their doctors. I also make sure my geriatric clients are socially engaged. Most importantly, I communicate with the family member who hired me.

Half of my practice is serving the needs of people with a wide range of mental health issues. We have assisted clients with bipolar disorder, depression, eating disorders, autism spectrum disorder, substance abuse and sexual abuse.
